Health and Safety Training

All new staff and volunteers must complete the following mandatory training in order to use the CHRI Facility. Some additional training may be required depending on your work at CHRI. Please consult your supervisor to see if you will require any additional training.

Once you are ready to take the quiz, you will be required to fill out a form before entering the quiz (see below). Please fill in the form with CHRI as your department and "I AM NOT A HOSPITAL EMPLOYEE" in place of an employee ID.

Privacy and Confidentiality

Privacy training must complete the following modules; M1, M2, M4, M5 - Non Clinical Support Staff. In section M5 - Non Clinical Support Staff complete the questionnaire and fill out the completion tracking form (see below). Under NO HOSPITAL ID, select Contract/Other as your employer, Western University as the Department and the Leader's name is Jennifer Devlin.

This training is mandatory for all radioisotope users.

This training is mandatory for all nuclear energy workers who have not received training in the last two years.

Radiation Safety (Nuclear) Awareness Training

This training is mandatory for all personnel who intend to enter a nuclear radiation room and have not received radiation safety training. Once this training is complete please sign appendix O and return it to the CHRI Admin office (A5-107).

Appendix O

Laser Safety

This training is mandatory for all class 3b or class 4 laser users. This training course is required every 2 years.

Transportation of Dangerous Goods (Ground Only)

This training is required for all University personnel who will be either shipping or receiving chemical shipments by land (road/rail) transport. This applies to both shipments sent by commercial means (ground courier) and to shipments driven personally (ie. chemical/samples driven off campus on public roads).

Biological Safety Cabinets

This training is highly recommended for those who work with biological safety cabinets in biosafety laboratories.

Faculty/Supervisors Responsibilities Seminar

This training is required for all University supervisors, principal investigators, administrative officers and managers who must know and understand what their role, legal duties and responsibilities are, as a supervisor, with respect to protecting the health and safety of those who report to them.

First Aid/CPR Certification

This course is reserved for permanent, full-time employees (excludes contract) who have been designated by their supervisor, and Occupational Health and Safety, as the official First Aid Representative for their department/unit.
